Change in Class and Status

Members who wish to transfer to the Inactive class of registration must download and complete the Application for Certificate in the Inactive Class of Registration. Please print and submit the completed form by mail to the CTCMPAO.

Members who wish to change from the Inactive class back to their previous class of registration must download and complete the Transfer Application for Certificate of Registration from the Inactive Class. Please print and submit the completed form by mail to the CTCMPAO.

For more information, please refer to the Notice to Members in the Inactive Class and the Reinstatement Process in the Policy for Certificate in the Inactive Class of Registration.

Members who wish to resign, must inform the College in writing including :

  • a declaration for your resignation; and
  • the reason and intention to resign; and
  • the date on which your resignation will take effect.

To request a change of name on the College of Traditional Chinese Medicine Practitioners and Acupuncturist’s public registry, you must complete the Name Change Request Form. You must also submit all required supporting documents for verification and a payment associated with the name change for printing of a new certificate.

Please note that it is not possible to change a registered name through the member portal.
