Members who wish to change from the Inactive class back to their previous class of registration must download and complete the Transfer Application for Certificate of Registration from the Inactive Class. Please print and submit the completed form by mail to the CTCMPAO.

Members who are authorized to use the Acupuncturist title and who wish to also use the title Traditional Chinese Medicine Practitioner need to complete and submit the Application for Title Variation Form and meet several conditions (please see the Application for Title Variation Guideline).
