Health Privacy Breach Statistics Reporting

Health privacy breaches must be reported to the Information and Privacy Commission of Ontario by March 1, 2019.

As a registered TCM practitioner and/or acupuncturist, you are a health information custodian and are therefore required to submit any health privacy breaches annually to the Information and Privacy Commissioner of Ontario.

The Commissioner produces a report identifying the frequency of personal health information that was stolen, lost, used without authority and/or disclosed with authority in 2018.

If you experienced 0 (zero) health privacy breaches during the 2018 reporting year, do not submit a report.

This letter and fact sheet from the Information and Privacy Commissioner of Ontario contains information on how to complete the report and includes the appropriate links to assist you. There is also an online statistics submission website open for health information custodians across Ontario to submit their statistics for the 2018 reporting year.

To submit your report, you will need a login ID. Contact the Information and Privacy Commissioner of Ontario office directly at statistics.ipc@ipc.on.ca to ask any questions and obtain your login ID.
